1 Check Your Package
- Tirzepatide Lyophilized Powder: Available in 10 mg, 15 mg, and 30 mg vials
- Bacteriostatic Water: 3 mL vial for dilution
- Syringes: Not included. Use 1 mL U-100 insulin syringes (available online or at pharmacies)
2 Choosing the Right Syringe
- Type: U-100 insulin syringe
- Capacity: 1 mL (1 cc)
- Gauge: 29G–31G
- Needle Length: 5/16 inch to 1/2 inch (8–12.7 mm)
- Use: Sterile, single-use only
Available at Amazon, Walmart, or most local pharmacies. A box of 30 typically costs $15–$25.
3 Preparing for Injection
- For 10 mg vial: Add 1.0 mL bacteriostatic water (final volume 1.0 mL)
- For 15 mg vial: Add 1.5 mL bacteriostatic water (final volume 1.5 mL)
- For 30 mg vial: Add 3.0 mL bacteriostatic water (final volume 3.0 mL)
Note: Units refer to markings on a U-100 insulin syringe, where 100 units equal 1 mL of solution.

4 Mixing Instructions
- Mix one vial of Tirzepatide powder with one vial of bacteriostatic water at a time.
- Use the entire mixed vial before preparing a new one to maintain stability and efficacy.
- Follow your healthcare provider’s instructions for dilution and dosage.
5 Dosage and Administration
Inject once weekly on the same day and time.
Administer subcutaneously in the abdomen, thigh, or upper arm. Rotate injection sites with each dose to minimize irritation.

6 Storage Instructions and BUD
- Lyophilized Powder: Store at 2–8°C (36–46°F). Use within 18 months from the manufacturing date.
- Mixed Solution (Reconstituted): Store at 2–8°C (36–46°F). Use within 28 days (BUD). Do not freeze.
Do not use if the solution appears cloudy, discolored, or contains particles.
7 Important Safety Information
- For subcutaneous injection only – do not inject into veins or muscles.
- Inspect the medication before use; do not use if the solution is cloudy or discolored.
- Follow your healthcare provider’s dosage instructions carefully; do not adjust the dose without consulting them.
